After Scheduling an Appointment

Please be sure and check the New Patient Forms page for the clinic. Filling out these forms before arriving at our office will help save some time.

What to bring to your appointment

Please have a photo identification and insurance card available. Please bring copies of previous x-rays, MRI's, or other diagnostic tests if applicable. We value your time and these items will help our ability to be thorough and efficient.  Additionally, please bring any New Patient Forms you may have filled out.

New Patient Forms and Policies

All new patient forms and policies may be downloaded from our New Patient Forms page. Please review, complete, and sign all forms where indicated. Filling out these forms prior to your appointment will save you substantial time. Please arrive approximately 20 minutes prior to your scheduled appointment time if you will be filling out the forms in the office.

If you are scheduled for an x-ray when making your appointment please arrive approximately 30 minutes prior to your office appointment time. This will again save you substantial time. X-ray is located in Suite 250 on the second floor. Turn left after exiting the elevators and look down the hall to your left.

X-rays may also be scheduled prior to your appointment date if you would like to save time at your office visit. They can be scheduled at multiple locations in the greater Austin area that may be more convenient for you. These x-rays can be viewed from our office. Copies of the films are not required. Please inquire about this option when making an appointment if you are interested.

Payment

Payment for any co-pays, deductibles or other fees due will be collected at the time of your appointment and may be made by credit card, cash, or check. We accept Visa, Mastercard, American Express, Discover and debit cards. Please see our Financial Policy for further details.
