Please bring copies of previous x-rays, MRI's, or other diagnostic tests if applicable.

We value your time and these items will help our ability to be thorough and efficient.

All new patient forms and policies may be downloaded from our Forms & Policies page. Please review the HIPPA Notice of Privacy Practices document. The New Patient Demographics, Patient Medical History, and HIPPA Consent Form should be filled out as completely as possible.

Filling out these forms prior to your appointment will save you substantial time. Please arrive approximately 20 minutes prior to your scheduled appointment time if you will be filling out the forms in the office.

If you were scheduled for an x-ray when making your appointment please arrive approximately 30 minutes prior to your office appointment time. This will again save you substantial time. X-ray is located in Suite 250 on the second floor. Turn left after exiting the elevators and look down the hall to your left.

Payment for any co-pays, deductibles or other fees due may be made by credit card, cash, or check. We accept Visa, Mastercard, American Express, Discover and debit cards. Financial arrangements can be made as necessary.
